The Short Answer

Cyberpsychosis is a serious rules-and-tone topic, not a shortcut to make an augmented character into a monster. The Core Rulebook connects it to Humanity, Empathy, dissociation, voluntary augmentation beyond the human baseline, and trauma. It also states plainly that cyberpsychosis is not necessarily violent. Use its mechanical thresholds accurately, but agree on the table's boundaries before the fiction gets personal.

For a player character, distinguish roleplaying guidance from loss of control. At lower Empathy thresholds, the book asks the player to portray escalating dissociative pressure. The character is handed to the GM only at zero Empathy with negative Humanity, and returns to the player when Humanity is positive again. Make that possibility explicit before it can surprise anyone.

Hot Take

Do not wait until somebody reaches a threshold to decide what cyberpsychosis means at your table. The best time to establish lines, veils, player agency, and what recovery looks like is before the first elective installation or traumatic job.

The caveat is that the rule can still matter. Removing every consequence makes Humanity feel cosmetic. The better approach is to make the consequence legible and consented to: strained connection, needed downtime, a clinic relationship, a crew check-in, or a hard choice about the next job - never a surprise excuse to make someone violent.

Read The Core Rulebook In Two Layers

The Core Rulebook contains both fictional framing and a usable mechanical procedure. Keep them separate so neither is flattened into a stereotype.

The Humanity Loss Guide covers the broader cyberware, trauma, and therapy decision. This guide focuses on how to run the cyberpsychosis material with accuracy and care once it approaches play.

The Thresholds Need A Clear Agreement

The Core Rulebook gives distinct guidance at EMP 3 or greater, EMP 2, EMP 1, EMP 0, and the extreme case of EMP 0 with negative Humanity. Do not reproduce those rules from memory or let the table discover them during a crisis. Open the book together and establish the expected procedure before the campaign reaches it.

The practical distinction is straightforward:

The values and full behavioral guidance belong in the Core Rulebook. What belongs in the session conversation is how those rules will feel at this particular table.

Cyberpsychosis Is Not A Violence Rule

The Core Rulebook explicitly says cyberpsychosis is not necessarily violent. It notes that violent outcomes are associated with particular fictional tendencies, not every person experiencing the condition. Preserve that distinction.

For GM practice, this means avoiding three common mistakes:

  1. Do not introduce a character with cyberpsychosis only as a combat encounter.
  2. Do not use real diagnostic labels as casual villain shorthand.
  3. Do not decide a player character's actions because their Humanity is low unless the book's defined GM-control condition applies.

Instead, show stakes through a player-approved signal: withdrawal from a trusted contact, a moment of disconnection, an impulsive but nonviolent decision, a clinic warning, a missed routine, or the crew noticing that their friend needs support. These create meaningful pressure without claiming that mental illness means danger.

Set Boundaries Before The Job Needs Them

The Core Rulebook itself recommends working out boundaries around difficult material. Cyberpunk RED can include body horror, surgery, trauma, addiction, coercion, violence, and mental-health themes. A simple pre-agreement makes the rules safer and easier to use.

Before play, answer these questions:

  • Are surgery and body modification on screen, veiled, or absent from description?
  • Are trauma-induced Humanity consequences part of this campaign, and who can call for a pause?
  • What language should the table avoid when discussing cyberpsychosis?
  • If GM control becomes possible, does the player prefer an off-screen transition, a jointly framed scene, or another agreed method?
  • What recovery resources exist in the campaign: a Medtech, a clinic, a trusted NPC, downtime, or a crew safety plan?

Revisit the answers when the campaign changes tone. Consent is not a one-time character-creation checkbox.

Make Recovery A Real Route

The Core Rulebook includes therapy that can restore Humanity. It requires time, care, and successful treatment, and installed cyberware can limit what Humanity returns without its removal. Those constraints are enough to make recovery consequential; the table does not need to add humiliation, forced violence, or hopelessness.

Use one of these approaches according to the group's preference:

The Cyberpunk RED GM Guide is the companion for turning that pressure into choices rather than a predetermined crisis. Put real options on the table, telegraph their consequences, and let the crew decide how it supports one of its own.

A Five-Minute GM Check

Before using cyberpsychosis material in a session, verify the following:

  1. The exact Empathy and Humanity state from the character sheet and Core Rulebook.
  2. Whether the book calls for player roleplaying guidance or GM control.
  3. The player's stated boundaries and preferred scene detail.
  4. At least two non-punitive choices for support, safety, or recovery.
  5. The difference between a difficult character moment and an excuse for an unwanted violent encounter.

Then state the stakes plainly. A table can handle strong material when it has informed choices, shared expectations, and a way back to player agency.
