The Short Answer

A Medtech makes injury, recovery, and medical access into choices the whole crew can act on. Medicine is not only an emergency button. It is a Role Ability with three specialties: Surgery for the most severe care, Pharmaceuticals for controlled medical effects, and Cryosystem Operation for stasis and its logistics.

Pick the kind of pressure your Medtech knows how to manage. Maybe they keep wounded runners alive long enough to escape, operate a trusted neighborhood clinic, build their practice around cyberware care, or carry a cryopump because their jobs always go farther than the crew expects. The recurring question is not "can we heal?" It is "what will survival cost, and who helps pay it?"

Hot Take

The Medtech should not make consequences disappear. They should make the crew choose which consequences it can survive.

A fast stabilization can buy an escape but leave recovery, treatment, debt, and the next job unresolved. A clinic can solve a problem but create a favor. Trauma Team can arrive for a covered client while someone else still needs a ride, a surgeon, or a safe room. When care has a price and a human connection, the Medtech is at the center of the campaign rather than waiting beside a hit point track.

Medicine Is Three Specialties

Whenever Medicine increases, the Medtech puts one point into Surgery, Medical Tech (Pharmaceuticals), or Medical Tech (Cryosystem Operation). Those choices decide what kind of care the character brings to the crew.

Medicine gives the Medtech a direction, but it does not eliminate the basic care everyone can attempt. The Core Rulebook distinguishes First Aid, Paramedic, Cybertech, and Surgery. That distinction is useful at the table: another character may keep someone alive long enough to move, while the Medtech handles the injury or procedure nobody else can safely solve.

Surgery Makes The Worst Injuries A Story

Surgery is available only through Medicine. It treats the most severe Critical Injuries and is also the skill used for cyberware installation, harvesting, bodysculpting, and therapy procedures described in the medical rules. A Medtech gains two Surgery Skill points per Surgery specialty point, up to the Core Rulebook's stated maximum.

Do not reduce that access to a quiet downtime roll. Give surgery a situation around it:

  1. What has to be treated before the character can safely return to the street?
  2. Where can the operation happen without interruption?
  3. What equipment, payment, favor, or cover does the clinic need?
  4. Who knows the patient is vulnerable there?
  5. What changes if the crew cannot wait for the ideal answer?

The Cyberpunk RED Cyberware Guide covers Humanity, prerequisites, capacity, and elective installation choices. This guide is the companion for deciding who performs the care, what it costs the crew, and how the table treats the aftermath.

Stabilization Is The First Decision, Not The Last

The Core Rulebook requires stabilization before natural healing can begin. First Aid or Paramedic can handle that immediate step; a Mortally Wounded person who is stabilized returns to one Hit Point but becomes unconscious. Severe Critical Injuries can still demand treatment, and the crew must still find time, safety, and money for recovery.

Use a short medical handoff in play:

This keeps the rules visible without turning injury into a lecture. The Medtech gives the group a clear medical reality. The players decide what they risk to change it.

Pharmaceuticals Reward Preparation, Not Panic Buying

The Pharmaceuticals specialty grants Medical Tech and access to specific medical pharmaceuticals as points are assigned. Synthesis uses the Core Rulebook's materials, check, time, and dose rules; these are controlled medical preparations, not street drugs. A non-Medtech cannot administer them correctly.

The valuable decision comes before the firefight. Stock the clinic or med bag for a real campaign pattern, then tell the crew what the supply is for. A dose that keeps a contact moving, purges a poison, or supports recovery is more interesting when the group remembers choosing it instead of producing a perfect answer after the danger is known.

Use the Core Rulebook for the exact drug effects, limits, materials, checks, and costs. Let the story decide whether the supplies are easy to replace, tied to a clinic, or the subject of a deal somebody will remember.

Cryo Creates Time, Not Safety

Cryosystem Operation grants Medical Tech and provides cryopump and cryotank benefits as the specialty increases. That makes it a powerful story tool: a critically injured person can be held in stasis while the crew finds money, treatment, transport, a donor, a surgeon, or an answer to a problem that cannot be solved before morning.

Stasis should never feel like an offscreen eraser. Ask practical questions. Who pays for the facility? Who can access the tank? What happens if the job moves to another city? Who has a reason to wake the patient too early, keep them frozen, or claim they were never there? The answers give a Cryo-focused Medtech recurring cases rather than a single piece of equipment.

Trauma Team Is An Arrival, Not A Resolution

The Core Rulebook describes Trauma Team as a specialized paramedical squad that retrieves wounded or incapacitated clients. Coverage can change an extraction, but it does not replace the crew's immediate choices. The client must be covered, the response changes the situation around them, and everyone else at the scene still has a problem to solve.

  • Let coverage matter when a covered character needs it.
  • Make the response visible: traffic, security, witnesses, rivals, and a sudden deadline can all change.
  • Keep non-clients and the crew's objective in the scene.
  • Let hospital care, recovery, bills, and obligations become future hooks only when the group wants that pressure.

Give The Medtech A Practice, Not Just A Kit

Choose two anchors that make the character part of Night City:

  • A clinic, ambulance contact, community space, or back room where care can happen.
  • A patient group the Medtech is determined to protect.
  • A supplier, hospital worker, or rival practitioner who can provide something rare at a cost.
  • A boundary around treatment that reveals what the character will and will not do for a client.

Those anchors give the GM material beyond combat. They also give other Roles a reason to help: the Solo guards the clinic, the Fixer finds supplies, the Netrunner protects patient records, the Tech repairs medical equipment, and the Media decides whether a hospital scandal becomes public.
